Title Assessment /Skilled Visit Nurse
Reports To Clinical Manager

Description/Responsibilities

Responsible for hands on nursing care in private home settings of patients of Northeast Clinical Services, Inc.consistent
with standard nursing practice and the policies, procedures and practices of NCS.

Job Functions

Provide direct care, support, and comfort to NCS patients and family members.

Assess and admit patients to NCS service consistent with home health care regulations and requirements
as well as the patient acceptance policies of NCS.
Make skilled intermittent home visits consistent with physician’s
orders and company protocols.
Provide skilled visit nursing consistent with physician’s orders and consistent with NCS practice.

Coordinate care plan development with NCS clinical administration and physicians.
Secure and implement interim orders from physicians as needed.
Quality Improvement: Participate in QAPI and utilization review as requested. Completes annual education on QAPI
Maintain proper clinical notes and documentation consistent with good nursing practice and NCS policies
Maintain appropriate communication with physicians and coworkers regarding the patients care.
Maintain open and direct communication with the patient and family members at all times.
Demonstrate clinical knowledge and judgment in the treatment of a wide range of medical conditions.
Prepare Oasis assessment form, 485’s and other required home health care documents and forms as may be required

     Participate in quality improvement and utilization review processes as requested
     Demonstrate clinical knowledge in practices and procedures relevant to their client’s care.
     Demonstrate a caring and compassionate demeanor at all times.
     Function with a high degree of independence requiring minimal supervision.

    Adhere to HIPPA rules and NSC confidentiality policies.
    Nurses’ appearance should be professional but casual.

 

Requirements

 

    • Valid and current licensure by the Commonwealth of Mass as a Registered Nurse.

 

    • A minimum of at least 6 months of experience in a hospital, nursing home or home health agency.

 

    • RN from an accredited School of Nursing required.

 

    • BSN preferred.

 

Knowledge Skills and Abilities

 

  • Current CPR certification strongly recommended

 

  • Ability to effectively communicate with patients, family members and co-workers.

 

  • Compassionate, caring personality

 

  • Ability to work independently

 

  • Knowledge of tracheotomy care and G-tube feeds, helpful

Physical Demands

The employee is frequently required to stand; walk; use hands to finger, handle or feel; and reach with hands and arms. The employee is occasionally required to sit, balance, stoop, kneel, crouch or crawl. The employee must frequently lift and move up to 10 pounds and occasionally lift and move objects up to 25 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.

Education

RN from an accredited School of Nursing required.

BSN preferred.
