LPN Case Manager – Day Habilitation Setting

Summary:

The LPN Case Manager is responsible to coordinate all aspects of health care for the individual in collaboration with the treatment team to ensure each individual achieves their maximum level of health and wellness by assessing, planning, implementing, coordinating, and evaluating options and services to meet an individual’s health care needs through communication and available resources to promote quality, cost-effective outcomes.

  • Participate in supervision, general staff meetings and trainings as scheduled.
  • Evaluate your assigned participant daily, in conjunction with the Health Care Supervisor, to determine medical needs.
  • Participate as part of an interdisciplinary team developing and implementing the Day Habilitation Support Plan (DHSP) for the participant to which you are assigned.
  • Support program staff members with direct “on-the-floor” support to program participants as needs arise.
  • Adhere to the assigned schedule specific to the program per Supervisor’s direction
  • Be able to present information regarding any individual in a professional manner in all settings including: meetings, phone contact, daily written documentation and /or email correspondences.
  • If utilizing agency funds must appropriately manage, obtain receipts for all purchases and adhere to Northeast Arc policy and procedures.
  • Adapt or modify program environments to ensure optimal independence when necessary.
  • Complete incident, accident and other reports in a timely manner as required by the Northeast Arc.
  • Adhere to the Day Services Division Personal Care Standard.(HSDS-20)
  • Provide assistance to individuals with all aspects of personal care which includes but not limited to toileting, mealtime support, assisting with hoyers, showering, and dressing.
  • Review and abide by each individual’s protocol outlining each individualized timelines and support requirements.
  • Administration of medications and special diets by mouth or via G-tube and maintain records of schedules of administration and side effects in accordance with Northeast Arc’s standards. (Note: licensed nurses may administer medication, paramedics do not administer medication; both may administer G-tube feedings
  • Ensure that all bathrooms/treatment areas are maintained in a sanitary manner.
  • Must be able to stand for extended periods of time, bend, push wheelchairs, and lift/pull to perform program and personal care tasks.
  • Perform all advocacy and case management responsibilities for assigned caseload
  • Complete and maintain written records as assigned by Supervisor: Prepare all paperwork as required by funding source for individual plans, maintain data, write and submit within the identified time frames progress notes or other documents as required.
  • Maintain confidential/medical section record as assigned
  • Flexible in job assignments/duties to ensure the safe delivery of services is available to people supported in any program within the day services division
