Personal Care Management RN

NORTHEAST CLINICAL SERVICES, INC. is a certified Massachusetts home care agency established in 2003. We have many medically complex infant to adult patients throughout Massachusetts in need of your Registered Nurse (RN) or Licensed Practical Nurse (LPN) skills, experience and compassion.

• Conducts initial face–to-face assessments with potential members in the individuals home or place of residence alongside an Occupational Therapist.

• Evaluates members’ physical and cognitive condition and resulting functional limitations to determine the member ability to benefit from and manage PCA services.

• Completes evaluation that accurately represent the member’s need for physical assistance with ADLs and IADLs. Completes time-for-task tool to identify time required for assistance with each task.

                            • Completes all required documentation accurately and in a timely manner.

• Communicates and collaborates with Occupational Therapist, Skills Trainer and other team members all pertinent communication to the PCM placement.

• Conducts re-evaluations, annually or more frequently if a significant change in the member’s physical condition or living condition has been reported.

                            • Other duties and special projects as assigned

Qualifications required for consideration:

                    •RN/LPN with current Massachusetts nursing license or reciprocity

                    •Current CPR and Massachusetts Drivers’ License

                    •Gtube, trach and/or vent RN or LPN experience ideal but will train depending on patient
