Summary: Provides oversight and manages the newly created Day Habilitation of the Northeast Arc. Develops a dynamic environment that supports the social and recreational goals of individuals supported within the program. .

Major Responsibilities:

•Ensures that programmatic goals and responsibilities are met, including but not limited to: Case Management, Case Record Management, medication administration, scheduling of daily activities, staff evaluation and training.

•Interviews, hires, trains and supervises staff. Addresses staffing disciplinary and/or terminations as outlined in Agency Policy and Procedures. Conducts staff evaluations and establish professional goals and objectives for each staff member annually

•Represents day hab program and NeArc in the community to develop relationships and collaborative partnerships.

•Evaluates assigned participants daily, in conjunction with the Health Care Supervisor, to determine medical needs.    

            •Performs all advocacy and case management responsibilities for assigned caseload.

•Completes and maintain written records. Prepares all paperwork as required by funding source for individual plans, maintains data, writes and submits within the identified time frames progress notes and/or other documents as required.

•Presents information regarding individuals in a professional manner in all settings including: meetings, phone contact, daily written documentation and /or email correspondences.

•Determines program eligibility of new referrals and make recommendation to individual’s team when they no longer meet eligibility and should be discharged.

•Coordinates transportation for individuals to and from community sites. Utilizes natural/community resources whenever possible.

•Coordinates and completes all tasks required to meet industry standards as established by Medicaid and CARF.

            •Maintains communication with residential programs, families and funding sources.

•Provides oversight to staff with regards to clinical and training needs of the individual’s daily programing schedules and behavioral programs, as outlined in their Day Hab Service Plans.

•Ensures all staff are trained in accordance with Northeast Arc policy and funding source regulations.

            •Functions as an active member of the Day Service Management Team.

            •Other duties and special projects as assigned.
