Job Summary: Provides direct medical support and skill training to individuals assigned participating in Day Habilitation. Provides back up administrative support to Program Director. Full time Monday through Friday 8:00am - 4:00pm schedule. No holidays, weekends or on-call required. Excellent benefits package with generous time off. In this rewarding position you will work as part of an interdisciplinary team overseeing the healthcare for adults with disabilities.

Major Responsibilities:

  • Oversees all health-care services provided to the member at the program.
  • Participates in the continuing interdisciplinary evaluation and reviews of each member’s needs and status.
  • Advices the Program Director and staff of any medical problems that may hinder a member’s progress or that may be aggravated by activities at the program.
  • Provides direct care and training in relevant areas including education in hygiene and health concerns.
  • Administers medications and treatments prescribed by the member’s physician during the time the member is at the program; (Administration of medication is in compliance with Department of Public Health regulations and Northeast Arc’s Safety Policies and Procedures regarding Medication Administration.)
  • Coordinates each member Day Hab Service Plan with other health care professionals including the nursing facilities where the member resides, if applicable.
  • Monitors each member’s health status and documents those findings in the member’s record at least monthly, quarterly as part of the interdisciplinary team review, and more often if the member’s condition requires more frequent monitoring.
  • Coordinates medical services with each member’s primary physician, obtains reports and approval of treatment plans from primary physician and reports changes in the member’s condition to the member’s physician; (see below for more information)
  • Oversees restorative therapy treatment as recommended by a therapist.
  • Coordinates implementation of physician’s orders with the member, family, and program staff while the member is at the program.
  • Reviews and abides by each individual’s protocol outlining each individualized timelines and support requirements.
  • Administers medications and special diets by mouth or via G-tube and maintain records of schedules of administration and side effects in accordance with Northeast Arc standards.
  • Ensures that all bathrooms/treatment areas are maintained in a sanitary manner.
  • Offers flexibility in job assignments/duties to ensure the safe delivery of services is available to people supported in any program within the day services division.
  • Performs all advocacy and case management responsibilities for assigned caseload.
  • Completes and maintains written records as assigned by Supervisor: Prepare all paperwork as required by finding source for individual plans, maintain data, write and submit within the identified time frames progress notes or other documents as required.
  • Maintains confidential records as assigned.
 
  • Education and Experience:
  • Registered nurse with at least one year of experience in a community health setting; and experience with multiple handicapped/developmentally disabled individuals, active Massachusetts RN license filed with the Human Resources Department.

Skills, Knowledge and Abilities: Ability to medically monitor individuals. Willingness to act as a driver as needed for transportation. Ability to write and communicate in English. Possess a valid driver’s license from MA, or neighboring states. Must be able to provide proof of good driving record. Have negative drug screen, CORI/SORI. Fingerprinting. Ability to work flexible hours.
